Patient Engage - simplify the patient choices to subscribe/unsubscribe

too many choices and for offices that are NOT doing birthday etc and just email and text of appointment the choices should just be text on/off, email on/off, voice on/off

    If the office is not sending newsletters, recalls, birthdays why should the patient receive a link to manage those choices? It is confusing to the patient. Only offer Opt in or Opt out of Email, Text, and Voice - period.
