It's also helpful to know if "4 quads" or "2 quads" are paid for srp and how long the covered "wait" is for the perio maint after srp therapy. Missing tooth clause, fiscal or calendar, waiting periods and oral surg medical/dental are some of the other common limitations that most offices need to know. Sealants are all over the place.
