Add Smoking Status from Medical History Review to Automated Clinical Notes

The Medical History Review automatically places most of the contents into the Clinical Notes, such as Medical Alert, Medications, Allergies, and Pain Scale. The Smoking Status is not included with this, and our providers would like it to be.
