Our sleep questionnaires forms will not accept the number zero. The forms should allow you to put a zero in and accept zero as a number. I have several patients that are unable to submit forms because they put the number zero. For example our question on the form is... How likely are you to doze off or fall asleep in the following situations. 0= I would never doze 2= I have a slight chance of dozing 3= I have a moderate chance of dozing 4= I have a high chance of dozing.
This won't allow our patient to do the paperwork online. Please get this resolved quickly. Thank you!!
