Allow more characters to be typed on statement notes

To print a manual patient specific note on a statement, it literally only allows a sentence and a half. Plenty of room to allow additional text.

Add report for Received Prior Authorizations

There should be a way to print out a report of all received pre-approvals. This way we have a list of patients to call who were approved or denied for treatment.
