Line by line patient payment option

It would be helpful to add patient payments directly to the procedure rather than just a date. Often, the patient wants a breakdown and this would be a huge time saver.

Close button on windows should be in the same corner consistently

When preparing claims, attaching documents, etc. Each window that opens has a different close button. Some have them in the upper right corner, some at the bottom. It would save time if this was consistent throughout and each window.
