Allow addition of the date when adding to the coverage table.

We should be able to have the date stamped on the allowed and paid amounts so we can keep track of it better.

On 7.3 Update, Dexis attachments aren't functioning properly for claims. Acquired date on xray attachment is auto-changing to today's date. Please Fix.

Change outstanding claims to different color.

All outstanding claim stays a certain color, but once its paid turns color. That way its easier to know which claim has been paid for or not.
