When setting a procedure code such as 27212 , which also asks for the lab code and fee, the procedure and laboratory codes should be separated on the claim form. Currently they are appearing on the same line showing ONLY the dentist code and not the laboratory code. Insurance companies require BOTH the dentist code and lab code to process a claim. When entering the procedure code, it prompts you to enter which lab code you will be using (99111, 99333, 99555) and the fee. Why does the claim not show the lab code and fee on a separate line? Insurance companies are not going to provide payment for a service they do not have a code for.
