NP New Patient and Non Patient should both be in the status of the family file

this will allow a family file to be created and still alert staff that it is a new patient

Add printed name on credit card to reporting

This information is helpful to determine if the name on the credit card matches the name of the patient. This helps with fraud investigations and to ensure credits are posted to the correct patient.
