CDT codes should display on walkout/ledger reports/billing statements

this would be helpful to patients with Flex Spending and HSA Accounts

Need to populate field 25 for codes that are for full mouth, like d0330, d1110, d1206 etc

The software is not populating field 25 for any codes that are related to full mouth being treated, like d1110, d1206, d0330 etc.
