Please add email address to Patient Route Slip

During this pandemic, email is the easiest way to reach pts. The route slip does not display whether this is on file or not. It is much quicker to look at the route slip then go into the family file to see if it is recorded.

We do full mouth ext and SDf. Those are more than 20 codes per appointment. We have to break this into multiple appointments to charge it all out. Inconvenient!
