fix all the modules in dentrix that requires selecting a new option in order to INACTIVE anything. Stop requiring the selection of a replacement item.

Show patient birthdate at top of patient chart

It is a bit cumbersome to have to get out of a patient chart to find their date of birth in the family file. It would be nice to have it show in the top bar next to the Patient Name.
