Lock the fee on a code

Sometimes we want a fee to stay a certain amount but updating fees in the treatment planner changes it. It would be nice to have a check box to lock the fee so it's not affected by fee updates.

Fast Check out options

It would be nice to be able to use the fast checkout and utilized the electronic insurance claim submission instead of just print or batch and then move to closing the claim if it is on assignment and the payment option, and print a walk out

Add "after the fact" note button to a closed month procedure so at any date you can make note of a tooth # change
