Add symbol to apt book if hyg not scheduled

So everyone in the office can quickly see if a patient who's scheduled for treatment needs to schedule for hygiene. Especially for offices with multiple providers.

Allow attachment of PDFs for supporting data for eClaim submission.

Patient alerts "do not show again today" should be workstation specific, not system wide

Just like health history alerts have a workstation-specific "do not show again today", so should general patient alerts. When one user doesn't need to see an alert that applies to finances, clinical issues, etc and clicks "do not show again today"...

Expand the guarantor notes character limits to grant more space
