The new forms don't sync in a timely manner and require EXACT information for them to sync. If there is one tiny detail that is different, then they don't sync. You cannot print the forms because the format changes once the forms have been filled ...

Opt-in Compliance issue with mydentist.com forms portal
The forms portal with patient engage needs a pop up before sending patient to the portal which allows them to opt in or out when they enter their phone number on the form in order to be 10DLC Registered and compliant.
