Make Payment Due notice pop-ups computer specific.

Scheduled payment due pop-ups should not be seen on all workstations. This is a front office task and disrupts the rest of the staff.

Opt out of email statements

We have some patients that still prefer to receive a paper statement. However if we have their email address on file the statement will deliver via email. There needs to be a way to opt out and still have email address on file.
