Forms you sell separately are available online, it's a silly extra step. Allow us to print the (HCFA212, e.g.) format with the claim. Someone suggested the same for a different format. Just because they sell these populated forms doesn't mean Dentrix which is dental software shouldn't at least have the option to print boxes on form.
