Add additional radial button for Inactive coverage and Termed coverage under Eligibility Status

Add additional radial button for Inactive coverage and Termed coverage under Eligibility Status.

We use the insurance eligibility button but if they are not eligible it just says other. I would like to see it turn ORANGE and show "Inactive" and have the option to change it to "Termed" which would change the color to RED so we are alerted to check with the patient for new insurance coverage. If you have questions just let me know.
