Create other formulas to calculate co-payment amounts
Allowed amt - Co pay X % coverage = patient fee
Allowed amt - Co pay X % coverage - pd amount = patient fee (this should work for plans that pay a PPO rate and we are allowed to charge the patient an additional amount because we are not a PPO provider.)
