When a new patient appointment is created and the patient is new, not established or linked to another family, I would like to see you add a slot for Date of Birth and Work number. Right now we have their names, Home and Mobile #, email address, mailing address and referred by. Please add DOB and WORK number to this box. Home number seems antiquated, not many have a home number, most everyone has a mobile number and a majority have a work number. Often patients are in a hurry to fill out patient forms and miss putting date of birth in, I would like to see it added as well as being able to view their Work number.
