New Patient appointment include more fields

When creating an appointment for a new patient, add the following fields: work phone, cell phone, preferred name, occupation, drivers license number, age/birthdate, referral source, insurance, emergency contact, special needs, medical alerts, drug allergies, note field, email address, etc. Would also like to be able to choose billing type and Fee Schedule. Would also like any of these fields not already in the family file to be added.

    Admin response

Thank you for the request. We added mobile phone and email to the New Patient Appointment information dialog in G6.

It would be great to allow for a box to add insurance information including member ID. I always have to type it into the appointment box.

It would also be nice if an addition to adding these fields if some of them could be REQUIRED fields to book an appt. At least the first & last name, DOB, and phone number.

Allowing for insurance would be a huge help!

DOB NEEDED. If NP is a minor, parent/guardian info section would be nice (ie: parent name). This could be prompted when the DOB is entered. Would love to have a section for subscriber info if not the patient themselves.

I remember the "good ole days" when we didn't need any of this info.  We just welcomed the new patient and put their name and number on the book.  Then we did the 1-2 page paperwork when they got here.  What was important was that they made an appointment to care for their teeth, and we were excited to help them.  Things have surely gone downhill since, and we're FORCED to get this info now because insurance companies are nefarious.  Next year, they'll add even more things.  Just wait and see.

Need muliple forms of contact for a new patient.... not just the one phone number. So in addition to email address need multiple phone numbers.

I AGREE. I AGREE. I wish a can get all the info on the new patient information section. Possible placing a section for notes for the Insurance information like the 800 numbers specially the State where the Insurance is located(like Delta dental , Blue shield and Blue Cross) New Patient information in addition of the address and phone, SSA and DOB. I do not want to make a new file and the pt. will not show.

birthday would be great too - since dentrix asks for birthdate to decide which teeth to default too according to birthdate

Or at least be able to attach xrays and insurance information to the appt. to then be imported upon family file creation.

I agree, I think it would save a lot of time later on. Especially the email, allowing them to receive reminders through eServices.
