We have a stamp with a template to guide the clinicians on what we need to know for the patient when they are being dismissed. For example T(for today) to write down what treatment was actually completed at todays visit and NDV(next doctors visit) for what they need to schedule for next. If we had a notes box at the bottom of the patient visit forms we could write our template in instead of stamping each form we print out daily.
