Veterans Affairs requires patient chart # (assigned by dentist) in box #23 to be filled in on all outgoing claims. Dentrix does not currently have this capability and we must print, write in & mail all claims and pre-determinations.

If the patient on the claim is the Subscriber, the claim will put a check in the box "Self" and leave the following boxes blank so as to not repeat the information above. For patients other than the subscriber, if the Relationship to Primary Subscriber (Family File |Primary Dental Insurance) is checked as Spouse, Dependent Child or Other, then box 23 will have the patient's chart number.
