It would be helpful to have the patient's address and insurance show up on the Health History form that prints from the Health History module (not questionnaire). We currently use the Health History info that prints when you hit the "printer" icon (Dentrix G7.2) at the top of the patient's Health History at their yearly exam for the patient to check and let us know if there are any changes that need to be added to their Health History. It would be helpful to have their address and insurance info on there as well so that they can let us know if either of those things have changed also.
