Show splinting on patient chart like you do for fillings and crowns

When viewing the appointment book, The preferred name should be in parentheses next to their name, not taking up a line in the appointment view.

Many appointments are only 2 units, not allowing to see the preferred name unless you "hover" over appointment.

Individual Color for Specific codes
