changing fee schedules

add a feature to change all assigned Fee Schedules for all patients at once. Example: all patients attached with Metlife can be changed to UCR because we are no longer a preferred provider with Metlife. Now we have to do it individually

Add a paint type for gingival grafting.

Add a "paint type" for gingival grafting. Something like you do for "watched tooth", but instead of a "W", make it a "G". It's a quick visual for the docs to see where there is grafting material
