time range for pre-authorization aging report

The report shows back to more than 10years ago information which is not helpful at all. We need to set time range for recent 12month please

The patient is able to either confirm or cancel their appt with one text. This will not reschedule the appt, but gives the office more notice of open appt they can fill.

claim form box 25 and 27

Box 25 should be automatically filled in based on the tooth number used in box 27. I have codes that are being denied because the insurance company for some reason wants tooth number AND area of mouth indicated.
