Add the attached fee schedule to the Insurance maintenance columns.

When looking up an insurance, It would be nice to see the fee schedule being utilized. It comes in handy when terminating insurance contracts, especially when some insurances use multiple fee schedules.

Put inital visit automatically as the first appointment

Right now, Dentrix considers the first visit the date the file was created and not the first appointment they are scheduled for. It should make the first visit the first appointment.

Assignment of Benefits default by plan

When we know a particular plan does not accept assignment of benefits, it would be nice to set that as the default when adding additional patients with the same plan, instead of having to look it up and uncheck for each patient.
