Medical History Improvement

When a patient completes the Medical history, have their responses show on the form in RED so they are easily visable.

Have the Payment table and coverage table work in matrimony.

This would make the treatment plans come out with better estimates when an office is out of network with an insurance and the insurance pays on a fee schedule.
