Please add signature & date line to Health History print out

In the Health History module, please allow a signature and date line when we print off HH forms for patient review.

Spanish online forms

I have a good number of Spanish patients and I would like to provide Spanish forms online. We have created our own health forms but the set up will not allow us to change the whole form.

If patient is booking directly online, at the end of the page the button where the patient clicks on should say "BOOK NOW" instead of "submit request" so they know they are not requesting to book but are already securing an appointment
