Signatures on new patient forms

Have patients be able to sign the forms when doing the paperwork online instead of waiting for them to come in and physically sign them that way they are completed and done before the appointment.

It would be really useful if hub 1 users were able to specify where they wanted specific forms to be downloaded to in Doc Center. Currently everything goes to "patient information" and then we have to remember to go back and move them to their app...
