Patient's copy of credit card receipt, remove signature section

I would the ability to enter last 4 digits of credit card # when entering patient credit card payment without having to use dentrix pay.

The patient and office would benefit when needing to refer back to source of payment.

Change the order of the buttons in Ledger and Family Files

The buttons can be rearranged in treatment planning, but not in the other modules/screens. Each person who logs in should be able to customize the toolbar buttons to their preferences.
