misc. note panel in chart

I think a miscellaneous note panel in chart for info such as; gags easily, prefers no cavitron, cannot recline, has a parastesia, etc. We should not have to"look" for notes in other modules.

Abutments should be able to be perio charted

Abutments currently change to pontics when perio charting which should be automatically skipped. However, abutments should be perio charted. This should not have to change the chart view of bridges as being connected abutment-pontic-abutment.

Perio chart BOLD center line

This is super simple, but would would make a huge difference for all the new people that come through our office. A dark solid line down the center of the perio chart so it's easy to differentiate when the Mesial and Distal change occurs.
