Invalidated procedures stay in the account ledger but it's very hard to see the +/* difference unless you're looking for it. Having invalidated procedures represented by a text color would easily eliminate a lot of confusion.

Better yet, give them a whole separate section - like the "display treatment plan" button in the ledger.
