adding spot for patient dob in "Enter New Patient Information" template

It would be useful to ask us to type in new patient's date of birth when scheduling a new patient as it is a crucial piece of info to identify any patient.

DOB needed in case there are two NPs with the same name and/or father & son w/ same name for example.

Also many offices verify insurance before seeing new patients, without the dob it is rare ins cos will give us info - we VERY OFTEN have to call back the NP & let them know we "forgot" to ask for their date of birth. Sometimes they do not even answer in time for us to get date of birth to verify insurance eligibility.

Front desk staff and patients would benefit from adding this feature. We would not seem incompetent to patients & the office could function more efficiently.
