multiple insurance companies bounce back claim submissions for D6190 requesting tooth #, quadrant AND arch. We use Box 35 to submit this info in the remarks, but their automated systems can't read this. The outcome is additional man hours required to print and mail in the claims with a narrative. This is a procedure code used often by periodontists and delayed payment has the additional impact on cashflow. We have been approached by at least one insurance company to use their online claim submission platform instead which is not a usable workaround for us since we rely on management reporting in Dentrix.
