When scheduling a new pt. not in the system, DOB is important information for identifying a patient that may have records at previous office. Instead of having to remember to ask and then add it in the notes after adding new patient info, it would be very useful to have that box pop up where you enter name, address, email and phone number.

yes and if patient is under 18, it would be nice to have another space added for parent info

This absolutely makes sense! We collect new patient name and contact information, DOB and insurance sections to the NP intake box would be immensely helpful!

Dentrix I-2332 suggests to add “New patient” as a status in the family file. I think this would solve most of these issues. You may want to vote for that.
