Customize Route Slips & Patient Visit Forms

We should be able to REALLY customize the Route Slips and Patient Visit Forms! I work in a dental office and feel that the Route Slips give too much information, but the Patient Visit forms just don't give enough. I like certain information boxes from each form, but we are not given the ability to mix and match the forms. In our office we need something simple, but that gives detailed information about that days visit. (Procedure, Procedure code, Description, Cost - Patient Name, DOB - But still a blank space that the clinical team is able to write notes if needed) To achieve this I would do a perfect mesh of the Route Slip and Patient Visit form!! But I've tried every combination imaginable within the separate forms to try to customize it to meet our needs as close as possible, and I can NOT achieve this... :( So very frustrating! It seems like it wouldn't be that hard to create a 'Custom Visit Form' that each office could then go in and select what information they want presented on the form...??? Please, please, please make this an option!

    Again, still need to be able to customize the route slips! Its now 2024. What's the status on this?

    On the patient route slip I'm shocked that the insurance information section doesn't include the the subscriber ID, DOB, or insurance company phone number.

    ALL medical history should print on Visit forms, including medications.

    Would love to be able to edit route slip. We do not want inactive family members showing up. We work the family recalls often and see who needs an appointment, but if a child is grown and is not a patient any longer, they still show up, even if the family is marked in-active or "insurance only".

    I would love to be able to give the patient visit form as a walk out but it either has too much information or not enough. The form is VERY busy and not easy to read.

    terrific idea !! so many ways we can use the data printed on each pt - This will make the route slip more useful each day .

    We had a power outage yesterday and were unable to contact some patients due to the fact that the Route Slip does NOT show the patients cell phone. We are a paperless office. IT is very frustrating that we can not customize the information that we would like to have on our route slip.  We also have been requesting a place for emergency contacts. That should also show up on the Route Slip. PLEASE make this update a priority.

    As a paper less office - A Patient Visit form or Patient Route Slip is the only printed paper for every one of our patients seen in our high volume pediatric dental office. To be able to Customize these forms or even have the ability to add a box for our own information would be greatly helpful!

    We use patient visit forms, and I print them on a form we created with completed treatment, future needs and a tooth chart. Works for us, but would love options. Great ideas!

    This would be EXTREMELY helpful! It would also be nice to add a place to circle if the patient needs referred out and if there is an expected Lab Return date to be noted for the front desk to know when the patient should be expected to come back.

    We need to have where on the patient visit form the information  below the word  Appointments is taken out due to our office puts notes there that the pt shouldnt see. We use this as a way for the pt to "update" ppw.  When you  print one now it says things such as

    1 wk call LVM gm 

    pt called and stated she had toothache on same area,etc

    pt is a christmas ornament 

     

    These are the things we dont want put on there when printed for following day. Please VOTE!!! We need all we can get!!!!

    We should have the ability to customize the route slips.  This is how a patient updates their information.

    We nee the ability to add email and customize our route slips to fit the scope of our practice.   this needs to be done ASAP and is long overdue

    There is an option to have the planned treatment at the bottom of the PVF in G6. I have it on mine! :)

    We recently upgraded from G5 to G6 and thereafter lost the feature to see the pre-printed procedures planned for an appt, which is specified at the bottom of the "PATIENT VISIT FORM: Patient Appts". Our office prefers the PVF to the PRS because Primary Dental Insurance coverage notes are available at a glance for Clinical staff AND Clinical staff then uses the space just below the "PATIENT APPOINTMENTS" as a redundant system to communicate to the Front Office staff edits to treatment planned for the day's appt AND which provider delivered said treatment. Having the planned treatment preprinted on the PVF form obviates the need for Clinical staff to handwrite everything and avoid additional time management concerns in the clinical setting. While this info is, in fact, listed at the top of the "PVF: Appointment", there is little space for clinical staff to write in edits to the appt and thus, lends to communication errors with Front Desk staff.

    We use G6 and it would be very helpful to edit the Route Slip. We want to print the mobile number for patients, not the H and W number, which many people do not use anymore.

    I really like that idea, and we would like to have the capability of doing the same thing at our office!
