Please Add a Signature for PT and Provider on the Health History app to lock date and health history changes, rather than only getting a signature to lock it in if they complete the online questionnaire that was sent to them. We don't want to send that to EXISTING patients. We want to update health history back in the clinical room. Then have them sign that one page so it locks in the date and any changes made. We are able to lock in health history when we transfer the health history to our notes, with the provider signature, but we need the PT to sign the health history verifying it is correct. If that PT signature can transfer to the PT chart then when we sign our clinical notes at the end of the day, both signatures will be in there and locked.
