Allow 3 signature boxes on electronic consent form. Medicaid requires us to have Patient/Parent signature, Dentist signature, and a witness signature. We are a multiple doctor practice so it would also be nice to allow you to choose the doctor performing treatment and autofill his name into the consent form and also allow you to choose the procedures the consent form applies to from the treatment plan rather than all procedures from the treatment plan printing on the consent form as we have different consent forms per procedure.
