Choose default provider for each Operatory

It would be nice to not have to change the provider each time you schedule a patient in the hygienist's room. Continuing care can be set for a specific provider, but it would be nice if ops1 and 5 could be set as their specific hygienists.

Option to have preferred name show up on patient communications.

Patients like it that we know their preferred name and call them by that name. It would be nice if our reminders reflected this.
