Default "balance forward" date for billing statements

Add the ability to specify the "balance forward" date on the Billing Statement; a relative time period, instead of always defaulting to 1 month ago. For example: 3 months, etc.

Show attachment ID

When calling insurance companies about a claim and discussing supporting information, they ask us for an attachment number. Without it being somewhere easy to see or accessible, it is harder to resolve issues
