Fix ledger freezing after sending bill

Ledger will freeze after approximately sending 10-15 bills and won't close. Have to restart computer to correct

Allow multiple providers to sign off on one clinical note.

Providers often collaborate during one patient visit (i.e. Hygienist and dentist). It would be very helpful if more than one provider could sign off on one note.
