Fix Patient Chart archiving incomplete treatment over 720 days old

Chart is hiding treatment planned procedures that remain incomplete after 720 days. Have been told this is a system flag that cannot be changed. This is a liability issue that must be addressed.

Adding Subject to Notes

when creating a clinical note it would be helpful to be able to name them instead of just a date "Recare" "spex exam" so at a glance you can see their history instead of sifting through notes.

Allow PDF uploads to claim attachments

Most of our forms are scanned in PDF which is not recognized by Dentrix. It would be more efficient to access PDF forms while submitting claims rather than trying to screenshot documents.
