Create a Report for Billing Statements

adjustments: allow/disallow certain adjustments per user

We would like to have each adjustment to be activated/deactivated per user. Certain adjustments (ie. Credit/Debit Balance Transfer, Insurance Adjustment) should not be allowed access to all users.

Add the ability to get production numbers by operatory

Certain ops are used for dedicated hygiene or surgery. It would be very helpful for us to select production from specific operatories, and not just by provider.
