Add ACH (check account paytment) to payment Agreement

Currently only credit card auto payment is supported. Please add ACH fuction to payment agreement, whcih allows patients to make payments directly from checking accounts.

Appointment confirmation should support https protocol.

Received an appointment confirmation e-mail from my dentist. Acceptance button refers to an http-only site at demandforced3.com. In this day and age they should certainly allow/accept https protocol.

Ensure information in comments box of dental claim forms is communicated in electronic submissions to insurance companies.

We can add information in advance that we anticipate the insurance company will ask prior to processing a predetermination, then they will have it on hand and not need to send a letter to the office.
