Add printed name on credit card to reporting

This information is helpful to determine if the name on the credit card matches the name of the patient. This helps with fraud investigations and to ensure credits are posted to the correct patient.

On line booking in Canada is actually a request to book an appointment as they will not sink with Dentrix. It also only looks at Active patient in the appointment book so it offers up times that are booked by inactive patients.
