Merging forms in Questionaire

We changed the idea of merging two forms together but I'd have to start over with the setup. It would be easier if we could just merge the forms together to save time .

Optional customized questionnaire option

With question such as "Have you felt ill within the past 14 days, if so please describe more" It would be nice to have the ability to make a yes or no option and right after it (staying on the right side of the paper) a word box allowing for input.
