Warn provider when E-billing needs updating BEFORE statements are created

If E-billing cannot be sent because of E-billing needing an updated credit card or User information on a new computer, it would be helpful to know that before statements are created and then requiring recreating after the update.

Add a billing fee for each monthly bill genersted

If you want to charge a billing fee there is no way to do that,, you have to go to each patient manually and add it, if you have 200 bills that is unproductive
