adjustments: allow/disallow certain adjustments per user

We would like to have each adjustment to be activated/deactivated per user. Certain adjustments (ie. Credit/Debit Balance Transfer, Insurance Adjustment) should not be allowed access to all users.

Be able to print a patient's ledger of payments only (over the counter) and print a patien'ts ledger without the running balance

Most patient's request a print-out of their payments for their taxes. Excluding the running balance on their ledger eliminates confusion for the patient

Ability to change the person that the tax is posted to

Prior to the latest update, tax was able to be posted to whomever made a purchase now it goes to the head of household. Please change it back!
